Information this website collects
You may browse this website without identifying yourself. The referral partner and support group inquiry forms on this site do not store or transmit your information through the website. They prepare an email in your own email app, and you choose whether to send it to CHI at contactus@choicesintl.org. Please do not include health information in these inquiries. Like most websites, our hosting provider may automatically record basic technical information, such as your IP address, browser type, device type, pages visited, and date and time of your visit, to operate and secure the site.
This website does not currently use advertising cookies or tracking pixels. If we add website analytics or advertising tools in the future, we will update this policy before doing so and will not send therapy-related or health information to advertising platforms.
Requesting counseling through SimplePractice
When you select “Request Counseling,” you leave this website and go to CHI’s secure Client Portal, which is operated by SimplePractice. Information you provide there, including appointment requests, intake forms, and messages, is handled through that secure platform and is governed by SimplePractice’s terms and privacy practices, as well as CHI’s obligations as a healthcare provider.
Your health information
Information you share with CHI as a client is protected under applicable federal and state health privacy laws, including HIPAA. How CHI may use and disclose your health information, and your rights regarding that information, are described in CHI’s Notice of Privacy Practices. To request a copy, please call 240-389-4685 or email contactus@choicesintl.org.
Email, phone, and social media
If you contact us by email, phone, or through social media, we use the information you provide to respond to your inquiry. Ordinary email and social media messages are not secure. Please do not send confidential health information by ordinary email, social media comments, or direct messages. Use the secure Client Portal or call our office instead.
How we use and share information
We use information collected through this website to respond to inquiries, operate and improve the site, and protect its security. We do not sell, rent, or lease personal information. We may disclose information when required by law, to protect the safety of a person, or with your permission.
